All personnel must be certified in lifesaving skills as a condition of working as a lifeguard. By signing this form, you agree and acknowledge that it is your responsibility to complete the lifesaving class online and in-water portions of the course. Certification Payment:

Please select from the packages listed below. If you are unsure of which link to select, please contact your hiring supervisor. After your payment is completed you should receive a notification that states your payment was made. *A minimum payment of $150 for the certification class is due prior to the beginning of the lifesaving class. All personnel as well as any returning Team Members needing a new lifesaving certification or re-certification will need to complete this form. Note: If a person departs from the company and their lifesaving certification is not completely paid for, the Company reserves the right to collect any remaining balance from monies owed.

If a situation arises that you are unable to afford your certification costs, a request for a special concession can be made on your behalf but must be approved by our Corporate offices. This request will work in the following manner: 1. Your payment to the PayPal account of $150 will be required to secure the certification class. The remaining balance would be divided into payroll deductions.

Additional Option

If you currently do not have your lifesaving certification and intend on taking the certification class with our Company, you can add this as an additional cost with your uniform order form and pay $157 deposit now. (That is $82 for your uniform and $75 deposit for your lifesaving class.) Note: Usually, a lifesaving class is about $250 – $350 for a 2-year certification. Also, this option cannot apply to any locations that require Ellis’s lifesaving certifications.

Additional Info

If you are currently not a certified lifeguard and intend on taking the lifesaving certification class with our Company you can add this to your uniform order payment for a discounted rate. If you add the lifesaving class now it is a $150.00 deposit paid now along with $87 payment for your uniform order (total $237).

IF YOU CHOOSE TO MAKE A CERTIFICATION DEPOSIT OF $100, YOU WILL BE PAY DEDUCTED AN ADDITIONAL $150. A CERTIFICATION DISCOUNT OF $150 IS ONLY AVAILABLE IF PAID IN FULL PRIOR TO THE START OF CERTIFICATION CLASSES.
